Network Office Patient Check in

The check-in form that was previously on this page is no longer in service.

You can notify Mobile Health the patient has checked in via one of the following:

  • Verify the patient clicked the link Mobile Health sent them via text or email confirming their attendance
  • Use the “Patient has/has not arrived” link sent to you in the automated attendance email
  • Send the exam number and check in time to network@mobilehealth.net
  • Call Mobile Health
