The Need
A large, Northeast hospital system needed qualitative respirator fit testing for thousands of employees across several sites in its network.
Objective
Deliver a large-scale, OSHA-aligned Qualitative Respiratory Fit Testing (QLFT) program to ensure workforce compliance across multiple hospital facilities and clinics, while accommodating a shift-based clinical workforce, minimizing disruption to patient care, and reducing operational and compliance risk.
Challenges
The challenges of executing this large-scale QLFT program were as follows:
- Testing a large, shift-based clinical workforce
- Limited staff availability to leave clinical units
- Tight execution timeline with high daily volume requirements
- Coordinating testing across multiple hospital locations
- Administrative burden and compliance risk for leadership
Solutions
We developed and executed a multi-faceted strategy to overcome the obstacles, including:
- Flexible, Shift-Aligned On-Site Testing: Extended on-site hours and variable testing schedules to support day, evening, and overnight staff.
- Mobile Pop-Up Testing in Clinic Areas: On-demand QLFT deployed in hospital and clinic locations across the health system’s multi-state service area.
- Rapid, High-Throughput Program Deployment: Standardized workflows and trained testers, enabling fast execution over a two-month period.
- Multi-Site Operational Coordination: Centralized staffing, logistics, and protocol management across all locations.
- End-to-End Program Management: Centralized scheduling, walk-in support, daily tracking, and monitoring reported to management to ensure success.
Results
High Participation
There were 13,705 total employees screened in less than 2 months.
Large-Scale Coverage
Multiple testing days over multiple shifts and across multiple sites minimized disruptions to patient care.
Flexible Deployment
Did 21 pop-up screenings in high-acuity areas, including OR, ER, and ICU to improve compliance across all departments.
Rapid Throughput
Each qualitative respirator fit test took 15 minutes on average, ensuring the rapid throughput necessary to meet the screening deadline.
