OPTION 4: SEE YOUR OWN MEDICAL PROVIDER
You may see your own medical provider for your required health assessment at your own expense. Print the Mobile Health Pre-Employment Physical Examination Form and have your medical provider complete it. You will also need to complete the drug attestation form. Then upload them using “Upload Documents” button below.

Option 5: POP-UP CLINICS
Mobile Health is bringing health assessments directly to select areas to help Personal Assistants meet their compliance requirements for the NY CDPAP. This option is at no expense to you.
ASSESSMENTS AT THE POP-UPS INCLUDE:
– Measles and Rubella Titer (Blood Draw)
– Health Assessment & Physical Exam with a Drug Attestation
– Tuberculosis Test through QuantiFERON Blood Test
– TB Risk Assessment
Please note vaccinations and confirmatory chest x-rays for tuberculosis are not available at pop-up clinics.

To continue working in the NY CDPAP, the New York Department of Health requires all Personal Assistants (PAs) to complete two forms each year:
- A Self-Health Assessment
- A Tuberculosis Risk Assessment or Survey
Mobile Health will email you two links to complete both forms online up to 30 days before the month they are due. For example, if your renewal is in July, you could receive the email as early as June 1. Completing both forms should take about 10 minutes total.
DON’T DELAY! BOTH FORMS MUST BE COMPLETED AND SUBMITTED IN ORDER TO CONTINUE WORKING AS A PA IN THE NY CDPAP.
Based on the medical review of your forms, you may need a follow-up procedure. If a follow-up is needed, Mobile Health will contact you to schedule it.

What is Mobile Health's role with PPL in the NY CDPAP?
Mobile Health is an occupational health company contracted by PPL to conduct all required health assessments for NY CDPAP Personal Assistants (PAs). They ensure that PAs meet all health requirements set by New York Department of Health before beginning employment.
Is there a deadline for completing my health assessment with Mobile Health?
For NEW Personal Assistants, once cleared by PPL, health assessments should be scheduled and completed ASAP.
For EXISTING Personal Assistants, health assessments were due October 1, 2025. PAs must make every effort to show up for their appointment or reschedule. Missing appointments will result in non-compliance and loss of eligibility to continue working in the NY CDPAP.
Can I go to another health provider to complete my health assessment?
If you complete your health assessment with Mobile Health, PPL will pay for it AND Mobile Health will handle all of your compliance documentation.Â
As a convenient option, you may also visit a medical provider at your own cost. Please print this Physical Form to take with you to your appointment. Have your provider fill out the form. You will also need to complete a Drug Attestation Form. Once complete, upload both forms here.

How is an EXISTING Personal Assistant (PA) defined by PPL and the NY CDPAP?
An existing PA is a Personal Assistant who is currently working or has previously worked in New York State’s Consumer Directed Personal Assistance Program and has already transitioned to PPL.
How do I schedule my appointment with Mobile Health as an EXISTING Personal Assistant?
Please note that Mobile Health cannot schedule any existing PAs who have not transitioned to PPL yet.
If you do not have any medical documentation to provide, you can schedule an in-person appointment, schedule a telehealth appointment, or visit one of our pop-up clinics.
You may also choose to see your own medical provider at your own expense. Please print and have them complete this Physical Form. You will also need to complete the Drug Attestation Form. Then upload both of those documents to Mobile Health.
If you provided medical documentation to PPL when you transitioned, our team is reviewing that. We will contact with more information and next steps if necessary.
What does the health assessment for EXISTING Personal Assistants (PAs) include?
 For all EXISTING Personal Assistants, the health assessment may include:
- A physical exam ensuring PAs are healthy enough to serve.
- Certification of immunity (called a titer, which is a blood draw) for Measles and Rubella, or the MMR vaccine (Measles, Mumps, and Rubella).
- TB test through a PPD skin test or QuantiFERON blood test, with a chest x-ray if results are positive.
- An annual TB risk assessment for negative cases.
- Drug attestation to ensure no drugs of abuse are being used.
It will depend on when you last completed a health assessment. The documentation you provide to PPL will determine which procedures are required.

Do I need to complete a new health assessment with Mobile Health if I'm an EXISTING Personal Assistant (PA)?
It depends on when you last completed a health assessment. The documentation you provide will determine which procedures are required at the appointment.

If I'm an EXISTING Personal Assistant, will Mobile Health test for things that weren't tested during my previous health assessment?
Possibly. Requirements may have changed since your last assessment. Mobile Health will ensure that your assessment meets all current New York Department of Health requirements for CDPAP Personal Assistants.
If I'm an EXISTING Personal Assistant, can I submit my existing medical records instead of doing a new health assessment?
You may submit your existing medical records using our Document Upload button above. Mobile Health will determine if your records meet current requirements, or if additional testing is needed and will follow up with you as necessary.
How is a NEW Personal Assistant (PA) defined by PPL and the NY CDPAP?
A new PA is someone who has never previously worked as a Personal Assistant in New York State’s Consumer Directed Personal Assistance Program and is being hired for the first time to provide home care services.
How do I schedule my appointment with Mobile Health as a NEW Personal Assistant?
After you have registered with PPL as a Personal Assistant (PA) for the NY CDPAP, AND received clearance from Mobile Health, you can schedule your health assessment. You will recieve an email, text message, and/or phone call from Mobile Health stating you are cleared to schedule your appointment.
You can schedule an in-person appointment, schedule a telehealth appointment, or visit one of our pop-up clinics.
You may also choose to see your own medical provider at your own expense. Please print and have them complete this Physical Form. You will also need to complete the Drug Attestation Form. Then upload both of those documents to Mobile Health.
Please note that Mobile Health CANNOT help you register with PPL, and CANNOT schedule your health assessment until after you’ve registered with and received clearance from PPL.
What does the health assessment for NEW Personal Assistants (PAs) include?
For all NEW Personal Assistants, the health assessment will include:

How soon can I get an appointment with Mobile Health?
Due to the high volume of screenings required by PPL for the NY CDPAP, appointment availability may vary. For the best availability, please schedule your appointment ASAP.
Where are there Mobile Health locations in New York?
Mobile Health has 40+ locations throughout New York City and 250+ across the state. You can select your location when scheduling.

Can I change my appointment?
Yes, you can change your appointment by logging into our Patient Portal. You can also call Mobile Health at 646-680-0450 to change your appointment. Changes must be made at least 24 hours before your scheduled appointment, and are based upon availability.
Can I cancel my appointment?
Yes, you can cancel your appointment by logging into our Patient Portal. You can also call Mobile Health at 646-680-0450 to cancel your appointment.
If you cancel, please reschedule your appointment ASAP if you still want to be a Personal Assistant.
What happens if I don't schedule or complete the health assessment?
You cannot work as a PPL Personal Assistant in the NY CDPAP.
ABOUT THE HEALTH ASSESSMENT
Why do I need a health assessment?
A health assessment ensures you’re physically able to perform the duties of a Personal Assistant (PA) and comply with New York Department of Health regulations. This assessment protects both you and the consumer you’ll be caring for.
Is there a cost for the health assessment?
If you do your health assessment with Mobile Health, PPL pays for it, so it is free for you.Â
If you go to another provider for your health assessment, you will be responsible for any associated cost.
What do I need to bring to my appointment with Mobile Health?
You only need to bring two things to your health assessment:
1. A government-issued photo ID, such as a state ID, driver’s license, or passport.
2. Your Mobile Health Confirmation email (can be printed or shown on your cell phone).
Do I need to fast for the health assessment?
No, fasting is not required for the PPL NY CDPAP health assessment.
Do I need my immunization (vaccination) records?
If you have them, please upload them to PPL when asked. This will speed up the process. If you don’t have them, Mobile Health will perform the required tests to check for existing immunity, which are called titers.
If I have documented proof of immunizations (vaccines) to I need to re-immunize (get the same vaccines again)?
No, if you have valid documentation of your immunizations, you will not need to be vaccinated again. Mobile Health will verify your documentation.
What immunizations (vaccines) are required?
Measles and Rubella: if no immunity present, the MMR vaccine will be offered.
Can I decline a vaccine?
Some vaccines may be declined for medical or religious reasons, but you’ll need to provide exemption paperwork. Please call PPL at 1-833-746-8283 for details on the exemption process.
How long will my results take?
Most results take 3-5 business days. Mobile Health will send them directly to PPL.
What if a health concern is found during my assessment?
Your clinician will explain any findings and may recommend follow-up with your primary care provider. PPL will contact you if there are any concerns that might affect your eligibility to become a Personal Assistant.
Do I need to do anything after completing the health assessment?
Sometimes, additional procedures are needed after an initial health assessment. If that is the case, Mobile Health will contact you. If no additional procedures are needed, you will learn from PPL whether or not you are cleared to work as a Personal Assistant.
How often do I need to complete a health assessment?
Health assessments are typically required upon initial hiring and may be required annually or if there are significant changes to your health status or job duties.
Who do I contact if I have questions about my health assessment?

Will Mobile Health notify me about my results?
No. Mobile Health will send your results directly to PPL. However, if there are any immediate concerns, they will be addressed during your appointment.
